Please read below to familiarise yourself with the number of players you are entitled to with your sponsorship, before filling out the forms at the bottom of the page. Please also take care to fill in each player's handicap and shirt size, which will be provided as part of the 2011 event.

Please note, golf carts must be hired from an external provider for the 2011 SEW Charity Golf Day. If you wish to secure one of these carts, please call the event team on (03) 9933 1000 before 9 September. Please note that late cart bookings will not be possible.
